I save my energy for the last heavy set of about 8-12 reps. Otherwise you burn yourself out before you get to push the heavy weights.
One set to failure for each excercise.

I save myself too as I time it for when I am the most warmed up and am used to the heavier weight then bang a failure set.
If the last set falls between 8-12 reps, cool but if it goes higher then cool too. I went to failure at a weight once and ended up getting 18 reps. Most ever at that weight. I could have gone up heavier but decided to go for it as a personal best.
The next week I just went up in weight.

i have written a long post with my anti-HIT viewpoint on training to failure (with facts to back up my argument) in the "advanced...." forum.
