Which plane of movement that you experience laxity (excessive movement in an unwanted plane) in the joint will help determine the type of problem you have. You say it's happened when twisting so it's possible this is a collateral ligament issue as opposed to a cruciate ligament. It's most likely to be the medial collateral that is either injured or has an inherent weakness. The cruciates offer some degree of rotational stability, but mainly restrict unwanted fore & aft movement at the joint of the tibia & femur. The loose body of cartilage floating round in the joint capsule is a separate issue, although it may well have been brought about by the fact that your knees are susceptible to this laxity.

I suspect, from what you have said, that this may be a condition you have either inherited, or you may well have damaged your knees as a child and it went undiagnosed. If you partially tear a ligament, it will be painful at the time of injury, but a complete tear will not be in a lot of cases. I'd get some exploratory surgery done as soon as you can.
