Sounds a little messy. I think you need to go back to a physio and stick with what they are telling you to do. I'd also be tempted to try a chiropractor. I had a dodgy left shoulder which caused me pain when raised to the front. Physio did very little for me, but after having the joint reset and adjusted by a chiro the pain went which in turn allowed me to build the muscles up again.
I'd been even more tempted given that the pain appears to run into your neck and spine.

Also - do you sleep on a hard mattress? My dad had a similar problem with a dead arm and it was due to his mattress cutting off circulation to his shoulder/arm.
