I dont know many people that inject outside on a road, in a smoke filled room, in a room full of people coughing, etc.

Drawing air into the barrel will not contaminate your gear.

If your sensible, you will be injecting after you have had a bath/shower and in a sterile enviroment (in the bathroom, bedroom) not somewhere when you have people coughing, car fumes or smoke!!
